Beetlejuice Wiki

Titan is a vast, endless desert landscape filled with sand dunes and massive brown structures of different shapes reaching skyward which serves as limbo, the space between life and death and a reference to the sands of time. It is the largest of the 274 moons of Saturn, the second largest planet in the solar system. Titan is inhabited by sandworms, which are giant worm aliens who have the ability to see ghosts and eat them as well as living beings. Adam and Barbara arrive here when they try to leave their home by 'normal' means after their death. This happens to Adam first in which he appears on Titan for a short time in which he is almost attacked by a sandworm until Barbara pulls him back onto the house's porch and tells him that he's been gone two hours, therefore, we can assume that, like the rest of the afterlife, time does not work the same here as in the living world. Adam and Barbara both arrive here again when Barbara gets upset about the new people in their house and are chased back to the door by another sandworm. After seeing Titan herself, Barbara finally realizes they are trapped in their house and can not leave.

In Beetlejuice Beetlejuice, the handbook gives instructions on how to open a portal to Titan, which Astrid does to defeat Delores. It can be assumed that the portal can also be used to open a portal from Titan back to earth as Barbara was able to bust through the ceiling of the house, but creates another portal on the floor as a sandworm crashes into the ground, so this may imply that sandworms themselves are capable of creating portals.
